How to refer a case or a diagnostic test?
1. Contact us on the phone 952 66 505 50 from 9 a.m. to 5 p.m. to speak with the hospital's Medical Director, the requested specialist or the authorized person in your department.
2. Download the form from the button below and fill it out with the requested information about the case or the evidence to be sent.
3. Send us an email to hello@hvbluecarepartners.es detailing the reason for the consultation and attaching the form and all the tests carried out so far.
